SmartCOPD (formerly EmmaCOPD)

Validated intervention that reduces hospitalization days by 79%

This care module includes the validated intervention SmartCOPD. SmartCOPD, formerly known as EmmaCOPD, helps patients recognize lung attacks independently. The intervention consists of a combination of the Digital Lung Attack Action Plan and the Activity Coach.

In 2018-2020, LUMC conducted research on the effectiveness of SmartCOPD in patients who had two or more exacerbations with hospitalization per year prior to the study. The results of this study show statistically significant evidence that SmartCOPD reduces hospitalization days by 79% in this target group. The results were published in April 2021 in the Journal of Medical Internet Research (JMIR).

If desired, the validated SmartCOPD intervention can also be used within primary care. For patients with a low disease burden, Viduet also offers a special configuration of the DigiLAP within this care module. Within this implementation, the digital lung attack action plan is activated as soon as the patient moves outside its bandwidths. In this way, the burden of the intervention on the patient is minimized.

DigiLAP (Digital Lung Attack Action Plan).

The DigiLAP is a standard part of the SmartCOPD care module, but can also be used as a stand-alone module. Many Dutch hospitals and general practitioners already use the paper lung attack action plan. The DigiLAP is a digital implementation of this paper lung attack action plan.

The caregiver completes with the patient what actions (such as taking medication or contacting the caregiver) the patient should take when symptoms consistent with a lung attack worsen. This digital implementation of the paper lung attack action plan ensures that patients better follow their lung attack action plan, allowing them to recognize symptoms faster and take appropriate action. In this way, the DigiLAP increases the self-management of COPD patients which can prevent many hospitalizations. Click on the link for more information on the DigiLAP.

Activity Coach

Coach the patient to adopt an optimal exercise pattern

Exercise is an important factor that can influence the disease course of COPD patients. Using the Activity Coach care module, you will ensure that your patient is coached to adopt optimal movement patterns. Optimal movement patterns help prevent exacerbations.

When you choose to deploy the Activity Coach, the patient will track their movement patterns through the smartwatch. Using our specially developed software for this smartwatch, the patient will be coached throughout the day to move within the ranges set by their healthcare provider, even if the patient moves too much.

This care module can easily be combined with coaching by a physical therapist. In that case, the physical therapist can set the optimal bandwidths and adjust them as needed. Through a smart algorithm, Viduet recognizes when the patient moves out of bandwidth for an extended period of time. Viduet can then also inform the physical therapist and patient in the desired manner so that the set bandwidths can be revised if necessary.

Oxygen saturation at home

Gain insight into the oxygen saturation of the COPD patient

If desired, through this care module, Viduet offers patients the option of having their oxygen saturation measured at home as well. The patient receives an oxygen saturation monitor linked to Viduet. Patients measure at agreed-upon measurement times and easily sync their measurements in the Viduet app. 

Once thresholds for readings are exceeded, patients and caregivers are immediately alerted in the desired manner. This way, appropriate action can be taken immediately.

COPD disease burden monitoring

With this care module, Viduet offers you the opportunity to monitor the disease burden of patients with COPD in a low-threshold manner. In this way, consultations can be used more efficiently and, in several cases, even saved.

This care module is particularly suitable for patients with a low disease burden (GOLD 1 or 2). Using validated questionnaires such as the CCQ or the Disease Burden Meter, the healthcare provider gains a better understanding of the patient’s disease burden. The patient completes the questionnaire at the appointed times in the Viduet app.

Viduet provides insight into trends of symptoms for both patient and caregiver. Because this questionnaire is completed each time prior to the consultation in the general practice, patient and caregiver are better prepared for the consultation. This not only promotes efficiency, but in some cases means that a consultation can be saved. In addition, Viduet offers the ability, through a safety net, to actively remind patients to make their checkup appointments. This can also save a lot of time in setting up chronic care for COPD patients.
